OVERALL CONFERENCE PURPOSE STATEMENT

Case managers are at the forefront of a patient’s transition across the healthcare continuum. There are many challenges we encounter within our complex medical and social cases compounded by a patient’s inability to access healthcare resources, make healthcare decisions due to cultural influences and the unpreparedness of their caregivers to provide support. The work of a case manager is often like a puzzle requiring innovative and creative solutions to successfully transition patients to next level of care.

 This conference will provide you with techniques to manage a complex medical case, and offer an important understanding of how to assess capacity for decision making by the patient.   Further discussion explains an understanding on how a patient and their family’s cultural spirituality can impact their health, care choices and ultimately the care delivery.   Lastly, the discussion emphasizes the necessity to assess the caregiver, and to assure the optimal level of care for the patient within the care plan.  We will equip you with the practical techniques, tools, and strategies for effectively managing your cases. You will gain a comprehensive understanding of capacity assessment in healthcare decision-making and learn to consider the social determinants of health, culture, spirituality, and a caregivers’ need for support. Session I – Innovations for Complex Case Management

This session will create a framework for case managers and healthcare professionals on how to drive innovation in managing complex patient cases.  To do so by enhancing patient outcomes by fostering seamless collaboration among multidisciplinary teams, ensuring effective coordination of resources and services, maintaining clear and consistent communication with all stakeholders and delivering comprehensive care tailored to the individual needs of complex cases.   Key highlights for all: focus on elements that contributes to modernized strategy and innovation in the following areas of the healthcare landscape:  capacity management, complex care, care giver support and critical conversations that lead to actionable strategies.  These elements are essential and interdependent on achieving optimized resource allocation, improved patient outcomes and successful case management outcomes across the continuum of care.

Session II – Spirituality in Healthcare: A Primer for Case Managers
How do we address spiritual influences that may impact an individual’s health, care choices and care delivery? Case managers acknowledge that person-centered care includes multicultural, religious, and spiritual factors. How comfortable are you assessing a person’s spirituality beyond “checking the box for religious preference” on an intake or admission assessment?  The “why” behind spirituality assessment is outlined in our professional organizations, Joint Commission, and other key case management partners. Join this critical conversation offering an overview of the essential role of spirituality in healthcare; essential definitions; assessment tools and practical applications for asking hard questions.

Session III – Navigating Caregiver Support: From Benefits to Assessment for Improved Outcomes on Complex Cases

We’ll also guide you through a mini caregiver assessment to tailor support for complex cases. Session IV – Assessing your Client’s Capacity for Informed Decision-Making
This session will provide information about factors that can impact your client’s ability to make informed decisions, including neurological and environmental factors. It will focus on different types of capacities and provide tools for you to ensure that your work with cognitively impaired clients is informed by ethical considerations.

General Chapter Event Registration Refund Policy
The following policy applies to all events:
• Registration can be transferred to another person if requested in writing by e-mail to CMSNE@CMSNE.org by the original registrant at least one week before the event.
• All refunds are subject to a 10% administrative fee. If canceled up to 7 days before the event, 100% of the registration fee (less the administrative fee) will be refunded.
If canceled less than 7 days before the event no refund is available.
• No refund is given for a cancellation on the day of the program or post event.

On the day of the event
• No refund will be offered at the venue if the attendee answers “Yes” to any of the screening questions.
