RN Case Manager
Anna Jaques Hospital has an excellent opportunity to join our healthcare team as a Registered Nurse Case Manager! In this role, you will develop a collaborative process that assesses plans, implements, coordinates, monitors and evaluates the options and services required to meet an individual’s health care needs. This includes coordinating/implementing discharge planning needs, adhering to policies on confidentiality, advocating for patient rights, working in accordance with regulatory agencies to maintain regulatory compliance, and providing quality and cost-effective outcomes for the patient populations.
