For the past ten years I served as a subject matter expert as the community transfer/travel Veteran coordinator and Minority Veteran Program coordinator at the Providence Veterans Medical Center, assisting and collaborating with community Providers in facilitating   appropriate transitions of care throughout the continuum.  My many years in nursing in the private sector and leadership roles in the military provided me with the opportunity to work with diverse populations in advocating and closing the gaps by providing best practices in care coordination.
