Overall Conference Purpose Statement

Steering Your Case Management Practice Toward Success will provide healthcare and case management professionals with practical professional and personal tools, which are designed to help navigate today’s rapidly changing environment.

The program will support the evolving role of case managers with emerging content updates including Dementia care in the home, the current vaccine landscape, regulatory updates in Utilization Review, collaboration on innovation strategies supporting healthcare, and a critical closing session on Case Manager wellness.

Participants will leave with actionable strategies to remain resilient, collaborative, and effective within today’s healthcare system.

Upon completion of this conference the attendees will be able to:

  • Describe the benefits of the GUIDE model of managing dementia patients in the home.
  • Discuss the national, local and ethical impact of changes to mandatory vaccine schedules through both patient and public health lenses.
  • Summarize the impact of recent changes in Utilization Management.
  • Examine the journey real-world innovators are navigating to advance Case Management practice and patient care.
  • Embrace and integrate Case Manager self-care tools to positively influence our own wellness.

Session I – GUIDE – Guiding an Improved Dementia Experience in the Home –
An Approach to Person-Centered Dementia Care

This presentation will provide an overview of the CMS Guiding an Improved Dementia Experience (GUIDE) Model and its role in supporting people living with dementia and their caregivers in the home setting.  Attendees will learn how the GUIDE Model promotes coordinated dementia care, caregiver education, care navigation, and access to community-based supports. The session will highlight practical considerations for case managers, including referral pathways, interdisciplinary collaboration, caregiver engagement, and  strategies to help individuals remain safely supported at home.

Session IV – Wellness – Its Not About the Yoga
Burnout, and lack of Occupational Wellbeing, have become endemic in the American Health Care System.  Traditionally, wellbeing was thought of as the responsibility of the individual healthcare worker.  More recent wellbeing literature has taught us that Wellbeing is far more about the Workplace, and includes Culture of Wellbeing and Efficiencies of Practice, in addition to these Individual Factors.  We will discuss the evolving science of Wellbeing, and talk more in depth about Culture, Values and Relationships as they impact our staffs Wellbeing.

This session aims to identify alignment between case management and technology-based platforms, highlight collaboration opportunities with innovators, and bridge knowledge gaps between case managers and tech developers.

Learning Objectives

  • Identify at least two common roadblocks in care transitions that can be addressed through collaboration between case managers and health tech innovators.
  • Describe how case managers’ workflow expertise can inform the refinement or implementation of technology platforms across the care continuum.
  • List specific types of information or feedback that tech founders need from case managers to ensure solutions support real world patient and family needs.
  • Apply a simple communication framework to articulate a workflow challenge to a technology partner (e.g., What’s the problem? When does it happen? What’s the impact?).
  • Compare the shared goals of case managers and health tech companies in improving patient transitions and outcomes.

General Chapter Event Registration Refund Policy
The following policy applies to all events:
• Registration can be transferred to another person if requested in writing by e-mail to CMSNE@CMSNE.org by the original registrant at least one week before the event.
• All refunds are subject to a 10% administrative fee. If canceled up to 7 days before the event, 100% of the registration fee (less the administrative fee) will be refunded.
If canceled less than 7 days before the event no refund is available.
• No refund is given for a cancellation on the day of the program or post event.

On the day of the event
• No refund will be offered at the venue if the attendee answers “Yes” to any of the screening questions.
