CMSNE Case Manager of the Year Award

No Case Manager works alone in providing the best possible care for their clients, they work cooperatively to coordinate services across the continuum.  With that being said, each day you interact with Case Managers who go above and beyond for their clients, exemplifying outstanding commitment and achievement in the practice of case management.   CMSNE would like to be able to recognize these case managers, those who not only go above and beyond, but who shine as true examples of what a Case Manager is.   Please considering nominated a colleague who is a CMSNE member or yourself.   As an organization we acknowledge the hesitation of members to recognize their own personal accomplishments, we want you to know that it is appropriate for Case Managers to consider nominating themselves for this award.  Consider working with a colleague or supervisor to submit an application. The candidate must meet the minimum criteria outlined below, as well as any additional evidence of excellence in the case management practice.  We invite and encourage members, colleagues, clients and other professionals to recognize an outstanding case manager and submit a nomination for the 2021 CMSNE Case Manager of the Year Award.  All the nominated candidates will be recognized at the 2021 CMSNE annual conference by the chapter membership and awarded recognition for their achievement.

CMSNE Award of Service Excellence

For many years CMSNE has used the Case Manager of the Year award to recognize an outstanding case management leader who has made significant contributions to the practice of case management. Building on that spirit of professional acknowledgement, CMSNE is pleased to offer a second award that recognizes excellence in case management practice.

The Award of Service Excellence recognizes an “in-the-trenches,” grass roots case manager whose day-to-day case management practice has transformed their field and contributed to the greater good – not only for their clients but for their community as well. CMSNE desires to recognize the extraordinary importance one individual can make through initiative, inspiration of others, and significant contributions to improving the quality of CM practice.  The evaluation process for this annual recognition utilizes Letters of Recommendation to determine the nominee’s contribution to:

  • Advancing case management
  • Exemplifying the Standards of Care and Standards of Performance as defined in CMSA’s Standards of Practice for Case Management
  • Demonstrating service excellence to clients and the case management community on a daily frontline basis
  • Incorporating innovation, creativity and resourcefulness in his/her case management practice
  • Serving others through community service by utilizing their case management skills in a non-practice setting.
